Registrar

The Office of the Registrar maintains student records and can provide credentialing information, including licensing, verification of medical education, diplomas, and transcripts or dean’s letters. Please read the information below and contact the Office of the Registrar if you have further questions.

Licensing Information

The Office of the Registrar is happy to assist graduates with licensure applications. Specific inquiries regarding licensure should be referred to the state in which you are applying.

You may forward your licensure application to this office for verification of medical education. If the license requires that you include an official transcript or Dean’s Letter, we must receive that request from you in writing with a live signature, authorizing the release of the documents. "Official" transcripts can be sent directly to graduates, or formerly enrolled students only if the final destination address is provided. The envelope, once received by the graduate or former student, may not be opened.  There is an average of a 7-10 business day turnaround time for all requests.

If the request also requires a certified copy of your diploma, you must provide us with a photocopy of your diploma. We will certify and forward it to the licensing board with your other documentation. If the application requires a photograph, please enclose that with your request.

Verification of Medical Education

Requests from program directors, hospital staff offices, etc. for certification of medical education may be mailed or faxed to the Office of the Registrar along with a signed release from the doctor or former student. As dictated by Federal Law, information regarding academic records can only be released upon written consent of the graduate.  The turn around time for all requests is an average of 7-10 business days.  We do not provide verbal verifications. We do not credential over the phone.

Loan Deferments

Please contact your lender for deferment forms, as they often vary. Please DO NOT COMPLETE ANY SECTION that says, "To be completed by the School Official."  Complete the student section and forward the deferment form and signed release to C-114.

Certified Diploma

Remove your diploma from the frame, take it to a copy shop and ask for black & white copies reduced to 75% of full size (color is nice, but not necessary), which will fit the diploma onto a standard 8 1/2 by 11 sheet in layout view.  Bring or send those copies to C-114 along with a completed credentialing request form. DO NOT USE ANYTHING HEAVIER THAN A 20 LB PAPER STOCK! Our machines do not process heavy stock, so we will be unable to certify your copies. (Common copier paper is perfect for this job.)

This office does not keep copies of student diplomas on file, prior to 2012.

To order a replacement diploma, download the Credentialing form, complete your information, sign and date the document, and send it with a check or money order made payable to Weill Cornell Medical College for $200 to:

Transcripts/Dean's Letter

The official grade transcript bears a raised Cornell University seal, the date of issue and the signature of the Registrar. The Office of the Registrar is happy to assist the Medical College alumni who are moving through post-graduate training or relocating, and require documentation of their enrollment at Weill Cornell Medical College or Cornell University Medical College.

Transcripts and Dean’s Letters, respectively, are sent directly to the program director, hospital staff office, credentialing agency, licensing board, educational institution, etc. An "official" transcript cannot be sent to the graduate unless it is in a sealed envelope and addressed to the final recipient. You may request an "unofficial" transcript for your records. It will be stamped "unofficial" and will not bear the school seal or signature of the Registrar.  Copies of Dean’s Letters are never sent to graduates.  If requestor requires more than one transcript, please attach a typed list of addresses.

Requests are processed within an average of 7-10 business days, from the date of receipt. If the requestor would like to expedite the mailing of the documents, via FedEx, please include a check for $20. The documents will be processed in the order in which they have been received, then sent overnight, via FedEx.

Due to the confidential nature of this information, we do not "Fax" Transcripts or Dean’s Letters.

Due to the Family Educational Rights and Privacy Act (FERPA) of 1974, we cannot respond to a transcript request without your legal signature.  Faxed signatures are permitted under law. The U.S. Department of Education has not yet ruled on the recently enacted electronic signature legislation. Until such time as we are advised how this impacts FERPA legislation, we are unable to honor transcript requests by e-mail, unless the request form has a live signature and has been scanned and emailed.

Weill Cornell Medical College reserves the right to withhold Transcripts, Dean’s Letters, grades, and diplomas from students who have unsatisfied financial obligations to the College.
