NEED-BASED AID consideration (grants):
The Office of Financial Aid will only send award letters to accepted / returning students. Waitlisted candidates are encouraged to submit the FAFSA and CSS Profile in advance, but should not submit any additional forms until they have received a formal acceptance from Admissions.
Required Application Forms:
- 2020-21 Free Application for Federal Student Aid (FAFSA school code: G04762)
- 2020-21 CSS Profile (school code: 2279)
- Aid Type Request Form 2020-21- Online form Waitlisted candidates, do not submit.
- Exemption from Tax Filing 2020-21 - Online form (ONLY for individuals who did not file or were not required to file a federal tax return)
- Signed copies of 2018 federal tax returns w/all schedules and W2 forms, including business forms from both student & biological parents (if filed) - UPLOAD Files Here!
- e.g. Submit complete federal 1040 tax form(s) (state forms are not required) with all W2 forms for all parties from year 2018
- e.g. Business owners must submit applicable forms: Schedule C, E, forms K-1, 1120, 1120-S, 1065, 4562, C-Corp tax returns etc.
- Waitlisted candidates, do not submit your sensitive tax documents until you have received your formal acceptance.
- Incomplete tax documentation will not be reviewed.

- Financial Aid Appeals Request Form 2020-21
- Monthly Living Resource & Expense Form 2020-21 (for families that reported low income, or if request by the OFA).
- Parent Abroad Form 2020-21 (for parents residing outside of the U.S. & do not file a US tax return)
- Parent Waiver Request Form - Online form
- Sibling School Enrollment Form for AY 2020-21 - (required for students with siblings enrolled in college full-time. Due by October 9th, 2020)

- Graduate PLUS Credit Check Authorization Form
- Online Loan Entrance Counseling & Promissory Note Completion is required for all first time federal loan borrowers - Select school as "Joan & Sanford Weill Medical College of Cornell University"
