MD Program Application Requirements

2024-25 Academic Year

OPTION 1: NEED-BASED AID Consideration (Debt Free Scholarship Program):

The Office of Financial Aid will only send award letters to accepted / returning students. Waitlisted candidates are encouraged to submit the FAFSA and CSS Profile in advance, but should not submit any additional forms until they have received a formal acceptance from Admissions.

Required Application Forms:

  • Signed copies of 2022 federal tax returns w/all schedules, including business forms (if filed)
    • Documents must be submitted for ALL: student, spouse, and both parents.
    • 2022 Federal 1040 tax form with all schedules (state forms not required)
    • 2022 Federal W-2s
    • Business owners must include applicable forms: Schedule C, E, forms K-1, 1120-S, 1065, 4562, etc.
  • Students will utilize the Financial Aid Self Service Portal- Netpartner- to submit all documentation and check application status. Netpartner can be accessed via LEARN portal.
Additional Forms that may be required in addition to the above:
  • Financial Aid Appeal Request Form 2024-25 (Form available in LEARN portal August 2024)
  • Monthly Living Resource & Expense Form 2023-24 (if requested by our office)
  • Parent Abroad Form 2024-25 (for parents residing outside of the U.S. &  do not file a US tax return)
  • Parent Waiver Request Form 2024-25 (For students requesting a waiver of one or both parents' information)
  • Sibling School Enrollment Form for AY 2024-25 (required for students with siblings enrolled in college full-time - Form will be released Late Spring 2024)
  • Student Bio Survey 2024-25 (required of all students who accept need-based scholarship support)

OPTION 2: FEDERAL LOANS Only (Direct Unsubsidized & Graduate PLUS loans):
