OPTION 1: NEED-BASED AID Consideration (Debt Free Scholarship Program):
The Office of Financial Aid will only send award letters to accepted / returning students. Waitlisted candidates are encouraged to submit the FAFSA and CSS Profile in advance, but should not submit any additional forms until they have received a formal acceptance from Admissions.
Required Application Forms:
- U.S. Social Security Number or Tax ID Number Information *IMPORTANT* This is needed to create your financial aid record (requires LEARN login)
- 2023-24 Free Application for Federal Student Aid (FAFSA school code: G04762)
- 2023-24 CSS Profile (school code: 2279)
- Aid Type Request Form 2023-24 (incoming MD only)
- Exemption from Tax Filing 2023-24 (ONLY for individuals who did not file or were not required to file a federal tax return)
- Signed copies of 2021 federal tax returns w/all schedules, including business forms (if filed) - UPLOAD Files Here!
- Documents must be submitted for ALL: student, spouse, and both parents.
- 2021 Federal 1040 tax form with all schedules (state forms not required)
- 2021 Federal W-2s
- Business owners must include applicable forms: Schedule C, E, forms K-1, 1120-S, 1065, 4562, etc.

- Financial Aid Appeal Request Form 2023-24 (Form available in LEARN portal August 2023)
- Monthly Living Resource & Expense Form 2023-24 (if requested by our office)
- Parent Abroad Form 2023-24 (for parents residing outside of the U.S. & do not file a US tax return)
- Parent Waiver Request Form 2023-24
- Sibling School Enrollment Form for AY 2023-24 (required for students with siblings enrolled in college full-time - Form will be released Summer 2023)
- Student Bio Survey 2023-24 (required of all students who accept need-based scholarship support)
OPTION 2: FEDERAL LOANS Only (Direct Unsubsidized & Graduate PLUS loans):

- Federal Entrance Counseling - required for all first time federal loan borrowers - Select school as "Joan & Sanford Weill Medical College of Cornell University"
- Federal Loan Master Promissory Note - required for all federal loan borrowers - Select school as "Joan & Sanford Weill Medical College of Cornell University"
